Initial Consult - Lily Langley
Client reports that Lily, a 2-year-old pug, was referred for hip dysplasia and back pain
associated with L4-5 calcification. When walking with a harness, Lily's front legs move
laterally, though her gait appears more normal without the harness. Lily seems to have
reduced flexibility in her forelimbs with the harness and has developed kyphosis in her back.
Additionally, the tail tone and position appear altered with harness application. Other
complications (brachiocephalic) makes solely collar use difficult. Some scooting behaviour
seen.
Gait Analysis:
Forelimbs move laterally when harness is on, more normal gait without harness.
Both hind limbs show external rotation mid-stance phase and elongated tarsal angle.
Left hindlimb is more exaggerated and of the left hind limb during walking.
The left hind limb is observed to move outside the sagittal plane.
Turning left and right was fair, weight bearing quality of hindlimbs was good and no
obvious instability was noted.
Postural Evaluation: Kyphosis noted in the lumbar spine. Hyper extended carpal joints from
cranial loading posture. Dislikes loading hindlimbs and will intermittently weight-shift.
Muscular Evaluation: Notable lack of biceps femoris muscle mass in both hind limbs, with a
weaker muscle tone. Also noted for quadriceps femoris and left hindlimb gracilis.
Hypertonic shoulder musculature was seen. Latissimus dorsi restricting forelimb protraction
and this was mildly resented. Trigger points and spasming in thoraco-lumbar junction and
around left rib heads. Neck and head musculature symmetrical and of good tone, balance
and coordination are good and in-tact.
