Charlie was seen for physiotherapy following referral and investigations with the Vets for suspected
PSSM. Chrlie returned without definitive diagnostics but did have some hot spots in his gluteals and
also a secondary test for type 2 PSSM was positive. In terms of subjective history, Charlie is not
barefoot and has displayed some sensitivity over rutted ground. His feet are in balance and look
good and strong.He has been developing groundwork sessions and working on Proprioception.
Posture –Altered since last visit, some mild cresty appearance over the neck and a drop in front of
the withers. Lost some muscle mass over the shoulders and a weaker topline and slighty dropped
thoracic segment of the back. Rounded and tucked gluteals and lumbar region and steep pelvic
angle. This is causing the hindlimbs to be further under the body (esp the left hind).
Gait assessment –Marginally more pelvic drop to the right hand side. Left hind lameness seen 1/10
only visible with very mild head nod in front. Lack of propulsion off the left hindlimb when in trot.
Tracks up well and walks with good rhythm, foot flight and placement.
Turning showed good hindlimb proprioception, placement, and foot height. Timing was good. Neck
looked flexible during turning, back showed visible tension.
Palpation –Spinal range of motion is improved and pelvic tilts show good pelvic motion too. Reduced
back spasming in total today but more on the left hand side. Still some tension over TFL left thigh but
also teamed with lack of quadriceps and intermittent locking of left stifle.
Exercises
Aim to engage the back muscles – Transitions between walk and trot (20 per session)
Tone muscles – Polework in walk and trot will increase joint mobility and tone the forelimbs
and stifle
Engage hindlimbs more – Work on mobilising the shoulders out of the way
