Thomas– Exercise Prescription and Notes

Subjective: Thomas was seem on his Christmas break. He has lost weight since the last
session and is looking lean with a more upright posture. Seems sensitive around his face and
head. A little anxious and reactive to surroundings, just moved back so may still be
adjusting.
Gait: Travelled on two tracks in walk. Pelvis drops down onto left hindlimb, has more tuber
coxae excursion on the right and has a wide gait with the hindlimbs with reduced hindlimb
flexion. Toe out left hindlimb and scuffs medially in stance phase, medial wall of foot worn
on this foot. Lateral wall of the right hind is scuffed. Close moving in front and narrow with a
toe out and lateral landing pattern on the left forelimb. Quick moving with low flight arc on
the left hindlimb and reduced weight bearing on the right hindlimb.
Main palpation findings: TMJ reactive and sensitive with restricted jaw movement left.
Right facial crest sensitive. Parotid glands lacking space and restricted as poll is restricted.
Tension across nuchal ligament and neck lacking in splenius development. Base of neck
reactive and hypertonic. Reactive on left rib heads T14-18 and into lumbar region. Left
subclavian slip of deep pectoral restricted and left caudal boarder of thoracic trapezius
reactive. Weak biceps femoris and quadriceps bilaterally. Lateral flare left fore foot.
Range of motion: Neck restricted c3-5 bilaterally, poll lateral movement good but over
used. Forelimb range of motion is good, hindlimb retraction limited left hindlimb.
