Patient Name: Poppy
Date: 4/11/24
Poppy was seen for her initial physiotherapy consult today following a
silicone implant for left hind patella surgery. She was seen post-op and the wound is
healing well, no stitches present and no signs of discharge or swelling. Poppy is bright and
alert and very active. Currently increasing walking by 5 minutes per week, on 20 minutes
at present. Good body weight
Subjective
Owner has seen Poppy using her hind-limb during routine walking, no scuffing,
dragging or issues immediately seen
In sit, Poppy hold the left hind-limb laterally
Crate used, access to stairways is blocked and main areas are carpet
No issues toileting or access to bed or routine care items
Can be stiff when rising in the morning
Objective
Physical Examination Findings:
o Left hind-limb Sartorius and quadriceps tenderness and slight contraction.
o Right hind-limb has increased muscle mass and tone and a medial position
when standing
o Reactive right semitendinosus
o Left hind-limb shows hesitation to bear weight; poppy appears to favour the
right limb during movement as shown by foot flattening
o Bilateral latissimus dorsi reactivity due to compensation
o Mild kyphosis of the lumbar spine
Range of Motion (ROM):
o Forelimbs: No issues, mildly resented right forelimb protraction
o Hindlimbs: Limited retraction on the left hindlimb, with crepitus in mid range,
not usual bone-bone feel, feels to be implant moving through range of
motion.
A: Assessment
Poppy shows reluctance to hold the left hind-limb in the sagittal plane to evenly
distribute her body weight
The hesitation to bear weight on the left hind-limb, combined with the altered sit
posture, over time will cause altered back biomechanics and increased strain on the
right hind-limb
.
