Bill - Physiotherapy Notes
05/08/24
Bill was seen for his initial assessment today His posture showed tension pattern and mild
tremors to the limbs – query anxiety or stress due to novel situation . Global under
utilisation of muscle mass. Shoulders show increased strain over lattissimus dorsi from
increased cranial loading. Long nails and some slippy floor may increase risk of injury –
advised runners in high traffic areas.
Head and neck – no abnormalities
Shoulders show brace lines and tension with associated trigger points and spasming in
latissimus dorsi muscle corresponding to forelimb loading. Mildly extended throughout
longissimus muscle in the back.
Lacking in gluteal which indicated hip extensors are not functioning – Bill has not run for a
while and so is under utilising hip extension movement. Hip extension range of motion
improved mildly following therapy but was still limited. Some mild crepitus heard on
occasion from hindlimbs on rising.
Functional tests: Sit posture -able to sit square. Rising from the sit required patience and
treats, able to push into stand and engage hindlimbs but unsteady with full engagement.
Back range of motion in dorsi-flexion is good. Proprioception is good.
Aim: Improve muscle tone, balance and support the joints. Take weight from the
shoulders and slowly increase onto hindlimbs
