Buddy Fisher – Clinical Notes

Subjective: Buddy was see following TPLO surgery for his initial consultation.
Functional tests reveal he is WB on all 4 limbs and able to sit, lie and walk
unaided. Toiletting and sleeping routine is good. Challenges are there are some
slippy surfaces in the home and 6 small flights of stairs to get to the flat.
Sitting: Straight sit posture, on rising still weight-shifts right and moves toes.
Objective: The left hind-limb has minimal swelling around the stifle and no
heat palpable.
Assessment: There was muscle reactivity around the thoraco-lumbar junction
and around the caudal scapula area bilaterally into lat. dorsi.
Crepitus felt on full flexion left stifle. Sartorius was reactive and painful on
palpation on the left and semitendinosus and biceps femoris on the right
restrictive of HL protraction.
Gait: Left hind medially placed. Tarsal valgus and toe out left hind. Good
rhythm of the walk, minimal scuffing, minor rhythm changes on the turn.
Plan: Therapeutic exercises, walking modifications and home modification

Subjective: No change to sleeping, demeanour or toilet habits. Buddy has
developed some reduced weight bearing on the LH following a walk this week.
Advised to follow with a vet visit to check lameness.
Sitting: Altered sit posture with increased lateral placement and stifle
extension of LH.
Objective: The left hind-limb has minimal swelling around the stifle and no
heat palpable.
Assessment: There was muscle reactivity around the thoraco-lumbar junction
and around the caudal scapula area bilaterally into lat. dorsi.

Crepitus felt on full flexion left stifle. Midial placement of LH in walk and stance
with decreased weight bearing evident from visual and leg lifting.
Gait: Left hind medially placed. Tarsal valgus and toe out left hind. Rhythm
slow and minimal LH movement on the turns.
Plan: Veterinary check

Subjective: The vet check revealed nothing additional to Buddys condition.
Advised to rest and monitor. NSAIDS prescribed and administered by owner as
directed. Buddy has improved with the period of rest and we are now building
back up to full function.
Sitting: Straight sit posture, on rising still some weight-shifts right minimal toe
movement.
Objective: Muscle mass measures 11 inches LH and weak on biceps femoris,
11.5 inches RH.
Assessment: There was mild muscle reactivity around the thoraco-lumbar
junction and around the caudal scapula area bilaterally into lat. dorsi. More
reactive right side lumbar longissimus.
Mild crepitus felt on full flexion left stifle.
Gait: Left hind straighter and almost placed sagitally (mildly medial). Tarsal
valgus minimal and toe out left hind. Good rhythm of the walk, no scuffing.
Plan: Slowly increase walking levels in accordance to plan 5 mins each
fortnight if no bad days. Continue exercises for function.
